How to configure select lists,radio groups and check boxes for a form

My council services provides the ability to add a select list, radio group and radio group to a form.




Administration->Service request and case->Forms builder



Once opened, please find the relevant form.




Select List


A select list is a list of values which the user has added to the form which the customer can select from when submitting the form. 

The customer will only able to select one answer from the list.


To add a select list to a form, you will need to add a new item and for the item type, you will select 'Select list"



Once you have selected the item type, you will then need to add a Placeholder and a Keyboard type which will be "List of values" 




When you have finished completing the Attributes section, please click on the add button.



You will now need to add your answers for the select list, this can be done by going back into the select list item you just created and scrolling down to the Answer section.





To add an answer, you can click on the add row button and then you can enter your answer into the Value field.


 You can add a Key for the answer but if you leave this blank then the system will generate a key for you.



Now that you have added your answers, please remember to click on the save button. 



Check box


A Check Box is a simpler alternative to a Select List in instances where Sub-Items are unnecessary. Check Boxes allow Form submitters to select any number of options from a list provided. 


To add a check box to a form, you will need to add a new item and for the item type, you will select 'Check Box"


For the keyboard type, you will need to select "List of Values"



Once this has been done please click on the add button. 


You will now need to add your answers to the check box, this can be done by going back into the check box item you just created and scrolling down to the Answer section.



As you can see, similar to the select list, I have added my answers to the item.



When submitting the form, the check box will look like this, you are able to select multiple answers. 




Radio Group


A Radio Group functions in a very similar manner to a Select List. Both provide a User with the ability to implement Sub-Items as many times as needed in order to create an intuitive chain of questions. The key difference between the two is that a Radio Group does not disable other options when the Form submitter selects an answer from the list, enabling the submitter to select several answers, if required.




To add a radio to a form, you will need to add a new item and for the item type, you will select 'Radio Group".


For the keyboard type, you will need to select "List of Values"



Now this had been added please click on the "Add" button.


You will now need to add your answer values by going back into the item and scrolling down to the answer section.


I used the add row button to add rows for my answer values.



When you have completed the answer values, please click on the save button.
