How do I configure form items for customer self-service scanning?

Designing the form for self-service scanning is quick and simple like the usual process.


The administrator can create a form using Forms Builder.


There are three plugin items to consider.




  1. Customer QR code




  2. Document types




  3. Web-service reference




Customer QR code plugin


The customer QR code plugin can be placed at the beginning of the form to retrieve the customer’s details thus reducing time and the need to input information.


At a later stage, the council could also use this further to include on pre-printed letters where the customer sends the letter as incoming mail with a QR reference which, could be scanned in the Mail Room and distributed digitally.


The following information is retrieved when the QR code is used.



  • FIRST_NAME

  • LAST_NAME

  • EMAIL

  • PHONE

  • MOBILE

  • ADDR1

  • ADDR2

  • CITY

  • POSTCODE


Note - Item code should be the same as above otherwise it will not retrieve the information.

Adding the plugin to form



  1. Add new Item to the form

  2. Select Item type as Plugin 

  3. Select "Customer (Scan)" from the list

  4. This will enable you. to fetch the customer details on the self-service scanning device using a QR code.

Document types plugin


This plugin will enable the customers to upload one or more documents as per the types configured by the administrator. 


Adding the plugin to form



  1. Add new Item to the form

  2. Select Item type as Plugin 

  3. Select "Advance file picker" from the list

  4. Select the document types from the list to be shown to the user while submitting the request

Web service reference plugin


This plugin is used to call 3rd party web services (see more on web service references).


These will be used to look up and validate identification information for example, if a customer enters their benefits number it can look up the value presented and check it is a valid reference number. 


Adding the plugin to form



  1. Add new Item to the form

  2. Select Item type as Plugin 

  3. Select "web service reference" from the list

  4. Select the Web service you wish to consume

  5. It will be used to look up to the configured 3rd party URL and display the details

Note*  the administrator must send the web services URL to support so we can add it to our whitelist and complete quality assurance.


Note*  Limitation – #REPORT_ID# (Dynamic field) and Calculative fields will not work in the mobile app.
