What is the minimum customer information I need to order a blue badge

CUSTOMER PROFILE (Community -> People)

The following items are the minimum requirements by the DfT when ordering a Blue Badge for delivery:

TITLE
FIRST NAME LAST NAME

DATE OF BIRTH
NATIONAL INSURANCE NUMBER (TAX IDENTIFIER) GENDER

FULL ADDRESS

- Please ensure that the post code is correct and properly formatted, e.g. NP15 1DN

CONTACT TELEPHONE NUMBER

- DfT only access the type of “Mobile Number”, which can be a landline if a mobile number is not available, but the type must be set as “Mobile”

EMAIL ADDRESS (OPTIONAL)

PHOTO

- A recent legible photo in portrait format (as a passport photo), showing a true resemblance of the Blue Badge holder, as this is to be printed onto the Blue Badge
