To add a page to your form, click on the button within the forms section to the left, or from within the Form itself.
The following should appear under the details section when “Add” is selected again, in the top right corner.
Select whether or not this page should be “Enabled” at this time. The page “name” usually reflects the details required by that particular page.
The other optional fields allow you to enter any information you wish to place on the “Header” and or “Footer” of the page, and description for staff purposes.
